NEVADA I am an out of network provider in Nevada.
I accept private pay and am happy to provide a superbill upon request, which clients can submit to insurance for possible reimbursement.
IOWA I am in-network with Wellmark BCBS in Iowa only.
*Some couples therapy options are NOT covered by insurance.*
Accepted forms of payment: credit card, debit card, FSA/HSA card.
Payment is due at time of service.
GOOD FAITH ESTIMATE
Under the federal No Surprises Act, healthcare providers are required
to provide clients who are not using insurance an estimate of expected charges for medical and mental health services. You are entitled to a Good Faith Estimate covering expected non-emergency healthcare and related costs.
